Ved å registrere denne Lipomic (Balanstri) testen samtykker jeg til følgende:

Prøven kan analyseres ved Lipomic Healthcare Pvt. Ltd. (LHC). LHC er et ISO 13485:2016 (medisinsk utstyr) sertifisert selskap med et ISO-17025 akkreditert laboratorium for måling av fettsyrer og HbA1c (langtids sukker) fra blod absorbert på et filtrerpapir (dry blood spot).

Jeg samtykker til at LHC kan bruke mine data til intern forskning. På vegne av meg og mine etterkommere fritar jeg LHC for ethvert ansvar utover avtalt analyseaktivitet.

Den registrerte hjemmetesten skaper ikke et doktor-pasient forhold, eller annen helserelasjon mellom meg og LHC. Resultatrapporten er veiledende og kan ikke benyttes direkte til diagnose, men som et supplement til en medisinsk undersøkelse.

By submitting to this laboratory test, you consent to testing for fatty acid analysis via Lipomic Healthcare Pvt. Ltd., accredited with ISO-17025 certification for conducting fatty analysis of the submitted sample. You consent to provide personal information such as, but not limited to, name, city and country of residence and e-mail address that may be required to communicate the test results via the Internet or email. You agree that your test results are not meant to diagnose, treat, prevent or cure any other health condition and understand that this data is not meant as medical advice, nor does it constitute any doctor-patient relationship between you and Lipomic Healthcare Pvt Ltd or any other party through whom you submit your blood sample for analysis at Lipomic Healthcare Pvt. Ltd.

On behalf of yourself and any representative or heir, you release Lipomic Healthcare Pvt. Ltd., or any other party through whom this sample is submitted, their affiliates/independent partners and any other entity or person associated with the test, and their respective successors, directors, officers, and employees, from any liability of any kind of nature.

PERSONAL INFORMATION COLLECTION

All personal information collected is done so exclusively with your consent, either through our website, or through personal information provided on the sample collection card. No information is collected automatically.

USE OF PERSONAL INFORMATION

As mentioned above, we use your personal information to appropriately process your requests and present you with the test report associated to this test. This information might, at a later time, allow us to add customized elements to our site or to plan its content more appropriately, based on user interests.

Lipomic Healthcare Pvt. Ltd. may use the data for internal research purpose or for the purpose of gaining Scientific and marketing insights that may be of benefit for developing new tests and products. Further, the knowledge gained can be used for writing Scientific or other news articles, or for the purpose of presentation at Scientific or other forums. In all cases, the information will only be used in ways that does not reveal who you are and not be identified in any publication.
